New
prescriptions:
Bring your new prescription in with your insurance card, or have your
physician fax it to us with a copy of your insurance card.
Switching your prescriptions to us:
Complete the Medication
Profile Form and fax or mail it to us. We’ll do the
rest and contact you when your medications are ready.
